Health care directive form Use this form to compete your own health care directive. This is a fill-in pdf form. Fill in the blanks, save the document, and print. Note the two ways to sign the document to make it legally valid.
Statutory short form power of attorney. This form, available from the Minnesota State Courts website, can be used to give someone else legal authority to handle some or all of your financial affairs. Print it and fill in the blanks. Be certain that you know what you’re doing, however, before executing a power of attorney.
