Estate Planning Intake Form Print and complete and bring to your first appointment.

Statutory Short-form Power of Attorney. This form can be used to give someone else legal authority to handle some or all of your financial affairs. Print it and fill in the blanks, paying close attention to where you and your attorney-in-fact must sign or initial. Be certain that you know what you’re doing, however, before executing a power of attorney.

Health care directive form Use this form to compete your own health care directive. This is a fill-in pdf form. Fill in the blanks, save the document, and print. Note the two ways to sign the document to make it legally valid.
